The Aladdin Theater, Portland, OR.  Opened in the 1920's as the Geller's Theatre.
The El Capitan Theatre in the Mission District in San Francisco, opened on June 29, 1928.  In the 1960's, they hollowed out the building and turned it into a parking garage.  I guess you have to desecrate a theater, that's a unique way to do it.

Opened in 1923, the Alexandria Theatre in San Francisco closed in 2004 and is in a shoddy state of repair.  There have been talks of restoration, but nothing has yet come to fruition.
The Avenue Theatre, opened in 1927.  The theater owners decided it was time to close in 1984, during a time when that section of San Francisco was marred by gangs and rampant drug use.
The Alhambra Theater, opened in the northern end of San Francisco in the 1920's, and closed in 1998.  It's now a fitness club.
