The Empress Theatre, Vallejo, CA. Opened in 1912 as the Empress but was renamed the Republic. After Fox West Coast Theatres took over, it became the Vallejo, then the Senator, then the Crest. Apparently, FWCT couldn't make up their minds. It closed in the early 1960's, but opened in the 80's as the Empress once again.
The Elmwood in Berkeley, CA. It opened in 1914 and has closed on occasion every since. At some point, it was modernized.
